The first step towards a beautiful, healthy smile is to schedule an appointment. Please call or text our office at 913-631-5600 to schedule, or complete an online appointment request.   

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download the new patient forms need at your first visit. Completing these forms ahead of time  will allow us to attend to your needs more quickly than completing them at your arrival.  Once filled out you may bring them to your appointment or email them to us at office@InHarmonyDental.Care
