Patient Forms

 Ah, the New Patient Paperwork...Good times!

If you are a new patient, as you are probably aware, there will definitely be some new patient forms. We do prefer these be completed prior to your dental visit, but to make it easy, you will be sent a link to fill these forms out on your computer or phone after you visit has been scheduled.  If you prefer actual paper, the forms can also be printed out and completed at home. You may bring the papers to your appointment or email them to us at Office@InHarmonyDental.Care

 If you have not completed  your new patient paperwork before your visit, please arrive 15 minutes early to complete the forms in-office as this will allow us to attend to your medical needs more quickly than completing them on your arrival.
